Any conversation you have with the therapist is confidential. The therapist wants you describe what you are feeling and will ask questions to clarify what symptoms you are presenting with.
The therapist will also perform a physically assessment before each session, determining what techniques will help you meet your goals and expectations. During the treatment he makes sure that you are comfortable by asking you for feedback.
Deep tissue is often not necessary for pain relief. Pressure is a minor aspect of dealing with painful or tight muscle tissue. When the pain is gone, pressure can be applied as much as is liked and tolerated.
